Before Your Appointment: Complete Patient Registration Online Forms. If needed, forms may be mailed to you when scheduling your appointment.
Important Reminder
All patients under the age of 18 must be accompanied by a parent or legal guardian at both the consultation and the surgery appointment.
Please bring the following documentation to your appointment:
- A valid photo ID - such as a driver’s license
- Medical and dental insurance cards
- A list of your current medications
- A list of any allergies

To help simplify your visit, we ask that all patients complete our secure online Patient Registration Form before their appointment. This form conveniently allows you to provide your information and electronic signature for:

What to Expect During Your Appointment

X-Rays & 3D Imaging
The Importance of Imaging in Oral Surgery
Accurate and current imaging is an essential part of oral and maxillofacial surgery. Dr. Williams requires a panoramic X-ray or, when indicated, a 3D cone beam CT scan to evaluate, diagnose, and plan a wide range of procedures. These include dental implant placement, removal of impacted or wisdom teeth, exposure of impacted teeth, and treatment for craniofacial anomalies, cysts, tumors, and jaw fractures.
High-quality images allow for precise assessment of tooth position, bone structure, sinus cavities, and nerve locations—ensuring safe, effective, and predictable surgical outcomes. X-rays are also used after surgery to monitor healing and confirm successful recovery.
We gladly accept recent X-rays from your general dentist or referring doctor if they are of diagnostic quality and were taken within one year of your scheduled surgery. Any images obtained in our office will be shared with your referring dentist for continuity of care.
Please note that dental insurance coverage for imaging varies by policy. Most plans cover one panoramic X-ray every three years and offer limited benefits for 3D cone beam CT imaging. We encourage patients to verify their coverage in advance.
